Meet Dr. Chan
Dr. Jacqueline Chan received her Medical Degree from New York Medical College. She completed her Internal Medicine residency in addition to a chief resident year at the former St. Vincent’s Catholic Medical Center. Dr. Chan began her Cardiovascular Disease fellowship at St. Vincent’s Catholic Medical Center and transitioned to New York Methodist Hospital to complete her clinical training. She is proficient in exercise treadmill stress test, echocardiography, nuclear myocardial perfusion scan, and peripheral vascular duplex. Dr. Chan completed training as a vein specialist and performs radiofrequency venous ablation in our ambulatory procedure lab. Currently, she is the Medical Director of Echocardiography and Vascular Imaging for our accredited ICAEL and ACR facility. Dr. Chan is multi-board certified in Internal Medicine, Echocardiography, Nuclear Cardiology, and Cardiovascular Diseases.

Office Visit Copays, Coinsurance, and Deductibles

Copay is typically a fixed amount associated with an office visit and diagnostic services, depending on your health plan agreement. Copay is collected at the time of service. Coinsurances are based upon the percentage of the allowed amount for a covered service; this portion varies depending on your healthcare contract. Coinsurance applies after deductible is met. Deductibles are predetermined and are also member’s responsibility. Deductible are an annual responsibility, defined by your benefit plan level, deductibles are also collected upfront.
